Blue Cross/Blue Shield of Michigan's Physician Group Incentive Program: June 17; 9:00-10:30 a.m. AST
Blue Cross Blue Shield of Michigan administers the largest patient-centered medical home (PCMH) program in the country, with a network of PCMH designated practices consisting of over 1,800 physicians in nearly 500 medical practices across their state serving close to 2 million Michigan residents. The program was launched in 2009 and has already demonstrated measurable results in strengthening quality, controlling costs, and managing patient care to improve health and prevent complications. The program has been recognized with a number of national awards, including the URAC National Quality Award for patient empowerment, and the Best of Blue Clinical Distinction Award.
