Fees-for-Service
The Alaska State Public Health Laboratories offer some services that include fees. Health care providers may use these services once a memorandum of agreement (MOA) is on file at the Alaska State Public Health Laboratories. Download and complete the necessary forms below for the testing desired and email to carol.archuleta@alaska.gov.
*Note: There is no charge for filing the MOA. However, it is recommended that MOA's are on file prior to submitting specimens to reduce processing delays.
Respiratory pathogen panel (RPP):
Includes simulataneous detection of respiratory syncytial virus (A vs. B), adenovirus, rhinovirus/enterovirus, human metapneumovirus, parainfluenza (1,2,3,4), coronavirus (NL63, OC43, HKU1, 229E), human bocavirus, Chlamydophila pneumoniae, and Mycoplasma pnemoniae from flocked swabs in universal transport media.
Herpes simplex virus PCR (HSV PCR):
Includes simultaneous detection and differentiation between herpes simplex virus 1 and 2 from flocked swabs in universal transport media.
Measles or Mumps PCR (MM PCR):
Includes either measles or mumps PCR testing for symptomatic cases, upon request.
Trichomonas NAAT (Trich):
Includes detection of Trichomonas vaginalis from urine, endocervical specimens, and vaginal specimens.
