State of Alaska Immunization Request Form and submitted by fax or mail.
- Requests must include a legible copy of a government issued photo ID that identifies the requester.
- Mailed vaccination records are sent by USPS 1st class and take a minimum of 5-7 business days to process.
- Individuals requesting a Yellow Card must include a note on the request stating that it is required for international travel.
- Records for minors cannot be released directly to parent(s)/guardian(s). Their records will only be released to a licensed daycare, school, or health care provider.
- Request records through your health care provider. Patients can request copies of their immunization records through their health care provider. All immunization service providers in Alaska are required to utilize VacTrAK to document vaccine administration. VacTrAK does NOT provide access to patients, only to health care providers.
